What we can do for Young Ophthalmologists

Bespoke is run by Young Ophthalmologists. We will share with you our experiences starting our own practices and get you to opening day and the painful few years that follow. We will pave your way to financial and professional success and offer support at every level, from macro to microscopic, from Day 1 to Year 5. Our consultants live in communities as rural as El Paso, Texas and as upscale as Beverly Hills, California.

We will directly fund your startup costs or help you secure competitive financing from a commercial lender in your area. We will discuss your career goals, practice needs, find you a beautiful space to use and then build you an aesthetically stunning practice

The details we worry about while you focus on working elsewhere to optimize your financial position prior to taking over the practice! Of course we will check in with you to discuss ceiling tiles, deconstructible walls, door choices (sliding or swing, glass or solid core, etc.), floor material (Luxury Vinyl Tile, Natural Stone, etc.), & much more.

But for the most part we leave you be while our architects and designers map out the space in AutoCAD and configure shelving, electrical, plumbing, etc. Sourcing building materials is a big part of Bespoke’s secret sauce. We don’t just go to the local Home Depot…we have  relationships with OEMs in China, Brazil, Turkey, Germany, South Korea, India, and Pakistan. A few times a year we hop across the way, rent a truck, drive from factory to factory in Shenzen and buy what you need as it rolls off the assembly line and gets shrink wrapped! We pack an FCL (Full Container Load) of equipment and building materials and ship it by sea to the parking lot of your practice where our contractors will unpack and install it! That’s as close to the source as you can get and it allows us to secure very competitive deals for all the beautiful & luxurious materials you’ll need to get started…and that’s what you need Bespoke for!

Beyond building materials we also source all the office furniture & medical equipment you could ever need so you can splurge on the things that matter, like Pentacams and lasers! Even there, we go outside the box to find you savings on things like the Pentacam table or phoropter hangars and exam chairs

Surgical equipment (sterile and non sterile) like Jewelers Forceps or iris dilators are all things we provide at great prices and excellent quality.

What we can do for Experienced Ophthalmologists

Once you get to opening day, your skilled hands & winning personality will start generating referrals hand over fist. In no time your balance sheet will be in the black and thereafter, the nature of your headaches and priorities will change.

One of the toughest pitfalls young practitioners struggle with is how to scale and how to delegate. While remaining focused on value & delivery of excellent patient care, you will now have the capital to further invest in your practice.

This may mean adding lanes, buying surgical equipment, opening a 2nd office or pulling the trigger on that ASC you’ve been dreaming of. But how can you do that without drumming up your overhead? Like a malignancy in your business, rising costs can take the gas out of your expansion plans & keeping that from happening is a Bespoke focus. We keep a sharp focus on containing costs while our Billing department ensures you get paid for the services you render.

Bespoke’s Architects digitize your clinic and ASC designs in AutoCAD, employing the “Copy Exactly” principle used by Intel to ensure all your offices and ORs have commonality of building design, materials, and layout. For routine maintenance , your support staff will know exactly where to find that spare microscope stand, where to order a replacement satin white 3.5 mm knobs on the exam room cabinetry, and so on. Following these principles, your future enterprises can gain  competitive advantage by being built to scale from Day 1.

Bespoke uses “Just In Time” lean manufacturing practices to manage inventory an help you keep pace operationally with even the deepest pocketed PE groups.

In the modern error, big enterprises no longer have a monopoly on Enterprise Grade management software. Bespoke employs big company solutions to help small practice operate more productively with ever fewer resources

Finally, Bespoke knows how much poor ergonomics can shorten a surgical career.  Problems with Kyphosis, Spinal Stenosis, and hand arthritis run roughshod through our profession and you can ignore them at your peril. Don’t be the 2500 case a year surgeon who must retire at 58, forfeiting the most lucrative years of your career, because your hands are shot and your back is killing you.

Bespoke offers a world class team of ergonomics experts including high-volume cataract surgeons, PM&R doctors, a kinesthesiologist, and a master Yoga Instructor who can visit you to do a physical exam to establish your baseline and then follow you around for a day or so before issuing their recommendations for equipment adjustments, more ergonomic furniture, slit lamp ocular adjustments. and an exercise and rehabilitation routine tailored to your physical exam results.

COntract Negotiations and Billing Management

Managing AI in a connected world

Insurance companies employ AI to test your billing processes, under paying on a few claims at a time. If no one calls to hunt down an unpaid claim, this is noted by the algorithm. Over several mths to a year, the program creates a profile of you that highlights your billing patterns and weaknesses. If your biller is meticulous about tracking payments for Prokera but perhaps you never charge for placing contact lenses…then over time that proclivity is reinforced; meaning if one day you DO bill for contact lens placement, the program will reject the claim, reinforcing your belief that this is an unpaid code not worth filing. As your profile solidifies in insurers’ databases, you come to find you are persistently underpaid on certain codes!

It is impossible to know to what extent payors share physician profiles, but certainly each company has you under the microscope of its own algorithm! Knowing that such devious practices are being employed against you, Bespoke employs expert billers to ensure you get paid for everything you do.

We structure your workflows, notes, documentation, and claim follow up to ensure any charting oversights are not repeated. We will find where you are leaving dollars on the table, and make sure you get paid for your services.

To the extent that uncompensated care happens; it should be on your own terms. The higher your rate of collections, the more freedom you will have to provide free care to those patients who most desperately need it

CUSTOM DESIGNED EDUCATIONAL MATERIALS AND PAPERWORK

Generic AAO pamphlets are nonspecific and impersonal. Introduce a high touch element to your practice by letting us create beautiful, branded brochures and educational materials that will help premium patients make informed decisions. Patients quickly tire of heavy handed sales tactics but will actively seek you out if your marketing focuses on educational handouts that highlight how premium products can improve their lives. Bespoke will adapt our templates to fit your practice needs and provide best in class branded paperwork of every sort, check in sheets, consent forms, billing sheets, and more. You get these forms as a standard service with unlimited revisions included. Bespoke will create customized, easily editable Op notes you can modify at any time. Our Reports are optimized such that you can spend less of your day messing with paperwork & more time operating!

Prior Authorization Management

Getting PAs approved is a challenge for any practice, from dry eye specialists trying to get Cequa filled to retina practices cranking out 100 injections a day. Although there are many billing services, there are no dedicated prior authorization services—until now. Bespoke will help you jettison personnel expenses by handling PAs for you. Our billing and PA services cooperate to find procedures you missed being paid on because of PAs & create protocols to ensure you get PAs for that specific CPT code moving forward. This is especially important for practices with heavy Medicaid populations.
